SNRI Antidepressants: Serotonin and Norepinephrine Reuptake Inhibition

SNRI Antidepressants are serotonin-norepinephrine reuptake inhibitors, a medication class that boosts serotonin and norepinephrine in the brain. You will see them for depression, anxiety, nerve pain, and fibromyalgia.

What SNRI Antidepressants Are

SNRIs block reuptake of serotonin and norepinephrine, increasing their availability at synapses. Common examples include venlafaxine (Effexor XR), desvenlafaxine (Pristiq), duloxetine (Cymbalta), and levomilnacipran (Fetzima). They are not DEA-controlled and are usually dispensed as maintenance prescriptions.

Why SNRI Antidepressants Matter for the PTCB Exam

The PTCB content outline expects you to recognize top drugs by brand/generic, therapeutic class, common adverse effects, and counseling points. PTCB items often use brand names to test generic recognition, so practice matching Cymbalta to duloxetine and Effexor XR to venlafaxine. Know that SNRIs can cause nausea, headache, insomnia, increased blood pressure, and serotonin syndrome risk with serotonergic drugs such as MAOIs, triptans, tramadol, and linezolid. Also know discontinuation syndrome can occur if stopped abruptly, so taper instructions matter.

Practical SNRI Antidepressants Application

If you receive duloxetine 30 mg delayed-release capsules, one daily, confirm the patient can swallow it whole and does not crush or chew it. For venlafaxine ER, remind the patient to take it with food and swallow whole. If a prescription looks like desvenlafaxine 50 mg daily, note it is not the same as venlafaxine and does not automatically substitute. Check for duplicate serotonergic therapy and document allergy/interaction notes accurately.

Common SNRI Antidepressants Mistake and Pro Tip

⚠️ Do not assume an SNRI is interchangeable with an SSRI or that patients can skip doses/stop abruptly; discontinuation symptoms can be distressing. 💡 Pro tip: when counseling, say “Do not stop this medicine suddenly—call your prescriber if you have side effects,” and reinforce the MAOI contraindication when relevant.

SNRI Antidepressants Quick Reference

  • Class: serotonin-norepinephrine reuptake inhibitors
  • Examples: venlafaxine, desvenlafaxine, duloxetine, levomilnacipran
  • Key risks: serotonin syndrome, BP changes, discontinuation syndrome
  • Counseling: swallow ER/DR forms whole; taper rather than stop abruptly
  • DEA: not federally controlled
